We require the minimum amount of information in order to comply with federal financial requirements. This information on your business includes:


  • Business Name

  • Business Address

  • Business Phone Number

  • Federal Tax ID Number (EIN)


The information required of all principal owners (with 25% ownership of the business or more)

  • Name

  • Address

  • Phone Number (Cell phone & home)

  • Social Security Number

  • Driver’s License (or Passport) Number


The documents we collect are:


  • Copy of Driver’s License

  • Copy of Business Debit Card

  • Certificate of Incorporation/Articles of Organization/Business Certificate

  • Filing Receipt (if applicable)

  • Proof of Federal Tax ID Number (copy of most recent tax return)
